ELYSIAN ANGELES LLC ( DEPILAIS EXPRESS) Memberships are not transferable and can only be used by the member. Membership payment cannot be refunded or used for the other services, regardless of usage. If payment is not successfully processed, membership will be automatically inactivated until payment processing failures will not be refunded or credited. Membership payment cannot be prorated. Members may obtain the benefits of the offers by scheduling a prior appointment throught our appointmennt service system or by attending withour prior appointment at DEPILAIS OR DEPILAIS EXPRESS
BILLING
All membership are activated on the day of your first appointment and automatically renew based on the member's billing cycle. Unlimited (montly) memberships are billed every month withour exception. Membership is activated on the day of the member's billing cycle begins,with that date being the day when the member sign's this form and make the first payment, once the member has selected the location(s) at which they with to receive service offerings. Membership must be paid in advance for the first month, your second payment will be billed one month (30 days) after your first billing. Membership is non-refundable. If a member loses the membership card, a replacement card will be issued at the location where you purchased your membership for an additional $7.
MEMBERSHIP CANCELATION:
Members may request cancellation of their membership at any time by providing fifteen (15) days written notice before their next billing cycle via our email: [email protected] Members will receive a cancellation form. Thereafter, we will process the cancellation once the member's cancellation form has been submitted. Cancellations are not processed in-store. Cancellations are not final unless our form is submitted within 15 days, you will be charged for your next billing cycle and will not be refuded. You will still have the ability to use your membership until the next billing cycle when you membership cancellation will be processed. Membership dues paid will not be prorated based on the date of cancellation and membership payment already paid will not be refunded. If the member shedules an appointment throught our appointment system and does not attend the appointment, the payment made for the reservation of the service will not be refunded without exception if the member cannot attend the appointment previously schedule by our reservation system must notify 24 hours in advance to the change of the appointment for another day of their preference.
For the cancellation form: if you do not require the membership any longer, and we received the cancellation form fifteen (15) days in advance, the form will be attached to the membership.
EXAMPLE: If the request was sent in the month of September, the cycle must still be invoiced in October and it will not continue to be charged the following month in this case November.
BILLING DISPUTES
Members will automatically be charged any bank fees assessed to ELYSIAN AGELES LLC (DEPILAIS EXPRESS), plus a processing fee of up to $25, for any payments returned due to closed accounts, insufficient funds, or other reasons. If this occurs, ELYSIAN ANGELES LLC ( DEILAIS EXPRESS) reserves the right to collect current and past due balances in any month after the returned payment. In the event a Member initiates a credit card dispute that is subsequently resolved in DEPILAIS ELYSIAN ANGELES LLC (DEPILAIS EXPRESS) favor, ELYSIAN ANEGLES LLC (DEPILAIS EXPRESS) shall be entitled to charge the Member or Pariticipant up to $35 per ocurrence. Members or Participants who dispute a valid charge for a late cancellation or no-show will be subject to this $35 fee.
CANCELLATION OF MEMBERSHIP BY ELYSIAN ANGELES LLC (DEPILAIS EXPRESS) :
ELYSIAN ANGELES LLC ( DEPILAIS EXPRESS): reserves the right to cancel your membership at any time with immediate effect if you are found to be in breach of the terms of the membership. All members are expected to treat our employees, other customers, and visitors with the utmost respect and dignity while at any of our locations. Any breach of this policy may also result in termination of your membership.
OFFERS:
To qualify for and Receive the benefits of our promotional and/or credit (DISCOUNT) Offer(s), you must meet the requirements stated, as applicable, in the Offer. You must have a valid account with DEPILAIS COMPANY, with a valid form of payment accepted on file to take advantage of the membership benefits (failure to validate these passed new members, customers will be disqualified and will not be eligible to receive Depilais Benefits). The company has a record of your first and last name, email, phone number, address, and credit or debit card that has been used for a previous purchase of a product or service at DEPILAIS AND DEPILIAS EXPRESS whether it is linked to an existing active or inactive DEPILAIS EXPRESS account or if fraudulent activity is suspected. Membership benefits cannot be combined unless otherwise stated. Terms of offers are subject to change by DEPILAIS CORP at any time. We reserve the right to modify or cancel any membership benefits, offers or discounts at any time. Membership benefits will be void when restricted or prohibited by law.
The clients understand that this authorization will remain in effect until. The client cancel it in writing, and agrees to notify the merchant in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. If the above noted payment dates fall on a weekend or holiday, The client understand that the payments may be executed on the next business day. For AHC debits or credit to their checking/savings account, the client understand that because these are electronic transactions, these funds may be withdrawn from their account as soon as the above noted periodic transaction dates. In the case of an ACH Transaction being rejected for Non-Sufficient Funds (NSF) the client understand that the merchant may at its discretion attempt to process the charge again within 30 days and then until de charge its succesfully completed. The client certify that ELYSIAN ANGELES LLC ( DEPILAIS EXPRESS) its authorized to used this bank account and will not dispute these scheduled transactions with their bank; so long as the transactions correspond to the terms indicated in this authorization form. This transanction will be placed monthly, as long as the membership is active
The client accepts this contract with ELYSIAN ANGELES LLC (DEPILAIS EXPRESS). Understanding that there will not be any kind of refund, under any conditions. The client must complete the payment of the membership monthly.
THE MEMBERSHIP FEE MAY BE SUBJECT TO AN INCREASE EVERY 6 MONTHS IF THE COMPANY DECIDE TO DO . WE WILL NOTIFY YOU BEFORE THE NEXT PAYMENT DUE DATE
